Initial Assessment

This is our first session together, and a time for us to get to know each other. It includes a review of your past medical history, goals, dieting history, current dietary habits and the formulation of an individualized plan that fits your life. We will work together to come up with a plan that is right for you. ~75 minutes

Follow Up

For returning clients. We will discuss your progress including any obstacles you may be dealing with. We will talk about what is working, what isn't, and re-evaluate your plan. ~50 minutes

Questions to Ask Your Insurance Provider

I encourage all new clients to call and confirm that medical nutrition therapy is covered under your particular plan. There should be a member services number on the back of your insurance card. Some questions to ask are:

  • Does my plan cover outpatient medical nutrition therapy? If so, how many visits are allowed?

  • Do I need a physician referral?

  • Do I have a deductible that I need to meet? If so, how much?

  • What is my co-pay amount for outpatient medical nutrition therapy?

If you have another insurance plan, I can provide you with a "Superbill" to submit to your insurance company for potential reimbursement. 

Payment is required at the time of service.
